External Case Manager

Woodland Behavioral and Nursing Center - We looking for an amazing, energetic person to work with our marketing team! As a critical member of the marketing, and admissions team, you, as the Hospital Liaison, will work closely with hospital discharge planners, physicians, social workers, case managers, and inpatient units to create a smooth transition for our current and future residents.
Duties and
Responsibilities:
Represent, organize, and execute marketing presentations within various communities Maintain relations with area physicians, social services departments, hospital discharge planners, governmental agencies, and other professionals Communicate with the same for the purpose of educating and building awareness of the facility's scope of services Coordinate all resident admissions including transportation, and obtaining proper admissions paperwork Assist families and residents in understanding policies and procedures of the facility and charges made for services rendered Responsible for following up with residents and families to ensure they are satisfied with our service Benefits- Paid Time Off Medical/Dental Voluntary Supplemental Insurance Plans Company Perks Employee Recognition Programs Team Like Atmosphere COVID-19 Precaution(s):
